The gut beats the calendar - microbiome composition predicts inflammation better than age

In the population-based DanFunD cohort, researchers analysed paired fecal microbiota profiles, 30 plasma cytokines and 19 biochemical and physiological measures in 1,199 adults aged 20-72. Gut microbiota composition explained more variance than chronological age for 97 percent of the cytokines and 84 percent of the physiological variables. People carrying the Bacteroides 2 enterotype showed an inflammaging-like profile up to 37 years earlier than other enterotypes, with a higher risk of later disease across multiple organ systems.

Key takeaways:

  • The signal is composition, not age: the same birthday can hide very different inflammatory profiles, and the microbiome read them better than the calendar did
  • Enterotype carried risk information: Bacteroides 2 tracked elevated pro-inflammatory cytokines and adverse metabolic profiles from early adulthood (hazard ratio 1.21 for incident disease), while a high-diversity enterotype tracked lower inflammation and reduced risk
  • Inflammation appears to be part of the mechanism: mediation analysis suggested inflammatory cytokines accounted for part of the link between enterotype and later disease

Hype check: Proven - an association, in one Danish cohort, between microbiome composition, inflammatory markers and later disease risk, with composition outperforming age as a correlate. Promising - the microbiome as a modifiable determinant of inflammation-related disease trajectories. Marketing - any claim that a commercial gut test, probiotic or transplant therefore lowers your inflammation or disease risk; no intervention was tested in this work.

Why it matters - This library tracks inflammaging across several entries (45, 49). This study moves the gut from background influence to a measurable scoreboard - a composition profile that flags inflammatory ageing decades early, in ordinary adults rather than patients.
